Was this scene fake or real?

In The Childhood of a Leader (2015), there is a scene where a 9yo boy is shown wearing a dress shirt. No pants or underwear and you see his penis. It's a British movie so I'm surprised they would have naked kids. Are those scenes showing naked boys in movies fake or real? I'm talking about recent, like 10 years or so, not from the 1960s.

I wonder how they prep the boy for the scene and how they can get away with it and no one threw a fit. And yes, Tom Sweet has a delicious sweet sausage.
